**Responsibilities**

**Community Patient Navigator**

**Northridge Hospital Medical Center**

The Patient Navigator will be a member of the Community Health and hospital health care team who helps individuals overcome barriers to COVID-19 vaccination. The Patient navigator will work effectively with both community residents and the multidisciplinary care teams and community partners. Duties to include:

+ Work with the Project Manager and Community Health Director to identify qualify and surveil collaborative partners and resources needed for increasing access to COVID-19 vaccination for community residents.

+ Responsible to have full understanding of community based resources and assist and track referrals to health-promoting community assets.

+ Participates in CHC initiatives that require registering community residents or tracking data (e.g. proposed “Vaccinate LA Collaborative”)

**Qualifications**

+ Minimum of 3 years of documented database application experience required. Expert level proficiency with Google Workspace and Microsoft Office Applications.

+ BS degree or equivalent college education and work experience required

+ Excellent verbal and written communication skills and information management skills required.

+ Excellent organizational skills required.

+ Bilingual Spanish preferred

**Pay Range**

$25.70 - $35.34 /hour
