**Responsibilities**

**Position Summary:**

Under direct supervision incumbent provides social services to patient and/or family in accordance with physicians orders. Incumbent acts as consultant to healthcare team and helps them better understand significant social and emotional factors as they affect the treatment plan. The incumbent reports to the Manager of Home Health.

**Qualifications**

**Minimum Qualifications:**

+ One year social work experience required

+ Two years of acute care hospital experience or a combination of hospital and other experience which provides for ability to fulfill the duties of this position preferred

**Pay Range**

$38.28 - $47.70 /hour
